What Should I Take With Me?

  • Savings Bonds you wish to cash
  • Proof of identity (check with your bank on what's acceptable)
  • A Certified Death Certificate for the owner if you are named as beneficiary (POD)

What if I Need Something the Bank Can't Help With?

  • You'll need to send us a signed FS Form 1522 (follow directions on the form).
  • Make sure you get your signature certified.
  • Mail unsigned bonds with the completed, certified 1522 (address is on the form).
